So many questions have been coming in lately about perimenopause, HRT, what to do, and when to do it from patients that I have yet to answer.

When should I start hormone therapy for perimenopause?

 

There is no single right time. It depends on your symptoms, your personal and family risk factors, and your long-term goals. For many women, the first signs are disrupted sleep and new anxiety, often linked to declining progesterone. If symptoms are affecting your daily life, that is reason enough to talk with your provider.

Is it too late to start HRT if I am past menopause?

 

Not necessarily. The timing hypothesis suggests that starting more than ten years after menopause or after about age sixty-five may shift the risk-benefit balance, but this is individualized. Feeling symptomatic is always worth a conversation, and delivery methods (such as transdermal versus oral) affects the risk profile.

Is hormone therapy safe?

 

For many women it is a safe and effective option when appropriately prescribed and monitored. Much of the fear traces to early interpretations of the Women's Health Initiative, which later reanalysis complicated. The increased breast cancer signal was linked specifically to a synthetic progestin, not to estrogen alone. Safety depends on your individual history, which is why proper evaluation matters.

How do I know if my symptoms are perimenopause or just stress?

 

A useful self-check: if you keep wondering whether what you feel is normal and you are struggling to get through your days, take it seriously. Rule out other contributors first, including thyroid issues, blood sugar imbalance, and anemia. If those are clear and your symptoms fit, it is worth discussing perimenopause with your provider.

When in my cycle should I test my hormones?

 

It depends on what you are evaluating. Days three to five are useful for fertility assessment. To look at the luteal phase, test seven to ten days before your expected period rather than on a fixed calendar day, since cycle length varies. Tracking your cycle for about three months first helps you time the test accurately.

​

Can food or supplements support my hormones?

 

They can help as part of a broader approach. Many women are under-eating, especially protein. Magnesium, B vitamins, adequate iron, and stable blood sugar all support hormonal health. There is no magic pill, and supplements work best alongside attention to nutrition, sleep, and stress.

​

Are at-home gut tests worth it?

 

Direct-to-consumer stool tests are difficult to interpret and often lead to overwhelming, supplement-heavy plans. Simple foundations come first: breathing before meals, chewing thoroughly, and trying a short period without gluten. If you pursue gut testing, do it with a provider who can interpret it properly.
